Appointments

Dr. Patel is available by appointment:
Mondays, Tuesdays, Thursdays & Fridays from 8:00am – 5:00pm Saturdays 8:00am – 1:00pm

Walk ins are welcome but appointments are preferred. Our staff is also available on Wednesdays to answer any office related questions. Dr. Patel will be paged in case of emergencies.

To schedule an appointment please contact our office at (713) 464-0236. Our Staff will be pleased to help you with anything you may need and we will make every effort to accommodate your schedule.
After you have scheduled your appointment, we will call you 24 hours prior to confirm your appointment. At that time, we will also make sure we have all of your current insurance and personal information to avoid any delays on the day of your appointment.

Please make sure you have the following items with you at your visit:

• A valid, active photo ID
• Insurance cards
• A current list of all medications you are currently taking with doses and frequency, including over the counter medications.
• A copy of any blood tests done at other physician offices.
• Co-payment, which is due at the time of your visit.

Cancellations

If you are unable to make it to your scheduled appointment, we ask that you kindly provide us with a 24-hour notice. This will allow us time to schedule another patient with more urgent needs. There will be a $25 charge for No Show or Missed Appointments. After 3 no show or missed appointments, patients will be discharged from the clinic.
